Course Exam
Proctor Agreement Form
If
your course requires proctored exams, complete this form the first
week of class. This will allow sufficient time to have the required
Midterm & Final Exam shipped to the Proctor.

SOCCOAST Afloat
ESO Cover Sheet
For
ESO’s to process registration requests for Afloat Courses. Coast
Guard member’s registration and TA Authorization forms must accompany
this form.

Online Drop
Form
For
students who wish to drop a course in which they are currently enrolled and are unable to do so using MyCCC.
Personal
Information Update Form
Use
this form if you need to request a change to your Name, Address,
Phone Number, Birthdate, or Social Security Number. Print the form,
fill it out, sign it, and fax or scan with necessary documentation
if applicable.

Scholarship
for Injured Service Members/Spouses Form
For
students seeking the Injured Service Members/Spouses Scholarship.
Must be submitted with supporting materials.

Math &
English Assessment - Proctor Agreement Form
For
students to secure a proctor and receive authorization to take a
Coastline assessment exam. Assessment/placement tests are recommended
for students who plan to pursue a degree form Coastline, and are
required prior to enrollment in an English or Math class.

Petition for
Early Transcript Posting
For students who completed 12sh at Coastline and need military credits posted early to their Coastline College transcript.

Request for
Degree Progress Update
For students that want an update or copy of their SOC Agreement/Degree Plan. Updates are provided once a year.

Graduation
Application Form
For
students ready to apply for graduation. Students are required to
petition for their degree at the beginning of the semester in which
they shall complete their final requirements. An official SOC Agreement
and official college transcripts must be on file before submitting
the application.

Petition for
Overload
For
students who wish to take more than seven (7) semester hours during an eight
week term, please complete, sign, and fax the Petition for Overload
form.

Transcript
Request Form
Students
may request transcripts online from the MyCCC Portal. As an alternative
to online services, students may complete this written form and
submit as directed on the form.
Registration
Form
This
printable Registration Form is provided for students who are unable
to utilize our online processes. Students in the NCPACE, GoArmyEd,
and eArmyU programs should utilize their respective program portals
for applying and registering.
For
any students EXCEPT eArmyU or NCPACE students who
are unable to complete the normal Online
Registration processes. This hardcopy faxed form is processed
only after Online Registration forms,
so students are encouraged to utilize the speedier online process.
Payment
Deferral
For
students who will be receiving Tuition Assistance, but will not
receive authorization prior to the end of the registration period.
Petition
to Repeat
For
students who wish to repeat a college credit class.
Request to Waive Academic Or Progress Disqualification
For students that are on "Disqualification Hold". Students must complete this form in order to register for courses and have the "Hold" cleared by a Counselor.
SOC Agreement/Degree Plan Request Form
For
students requesting an official evaluation of military and/or college credits towards a degree at Coastline must complete this form. Upon enrollment and submittal of official military and college transcripts the SOC Agreement/Degree Plan can be provided.
Request to Line-Out a “D” or “F” Grade (pdf)
For students who have repeated a course prior to Spring 2008 and would like the previous grade of "D" or "F" exempted from their GPA calculation. Students repeating a course after Spring 2008 do not need to complete this form; the previous course will automatically be exempted at the end of each semester or at the time of graduation.
